Apply to the Mental Health Court

The application process begins with a referral from your attorney. Review the eligibility criteria and what to prepare, then complete the application below.

Four steps from referral to enrollment.

Your attorney initiates the process — but you can begin gathering information and completing the application below at any time.

Attorney Referral

Your attorney submits a referral form to the Mental Health Court along with supporting documentation about your case and diagnosis.

Complete Application

Submit the full application below. All fields are required. Be as thorough as possible — incomplete applications delay review.

Court Team Review

Coordinator Skylar Boles and the court team review your application within 3–5 business days and follow up with you or your attorney.

Enrollment Decision

If accepted, you and your attorney will complete a formal program contract. Your 18-month program begins from that date.

Are you eligible?

Review these criteria before applying. If you are unsure whether you qualify, contact Skylar Boles at 770-443-7541 — do not self-select out without speaking to the coordinator first.

✓  Generally Eligible

  • Documented diagnosis of a severe and persistent mental illness
  • Current or willing-to-become Paulding County resident
  • Mental illness directly correlates with the current offense
  • Moderate to high risk of reoffending without treatment
  • Willing to voluntarily participate and comply with all requirements
  • No active immigration holds
  • Represented by an attorney who can submit a referral

✗  Generally Ineligible

  • Active immigration holds or pending deportation proceedings
  • No documented mental health diagnosis
  • Unwilling to voluntarily participate or comply with treatment
  • Current offense not correlated with mental illness
  • Certain violent offense histories — reviewed case by case

Eligibility is always determined on an individual basis. If in doubt, apply — the court team will make the determination.

🧠 Mental Health Documentation

  • Documented mental health diagnosis from a licensed provider
  • Current treatment provider name and contact info
  • Current prescribed medications
  • Prior hospitalization history if applicable

Mental Health Court Application

Complete all required fields as thoroughly as possible. Incomplete applications will delay the review process. The court team will contact you or your attorney within 3–5 business days of submission.
